Personal Information 1

Surname / Family Name

Exactly as it appears on your passport. If you have only one name, enter it in Surname and check "Does Not Apply" for Given Names.

Given Names

All given/first/middle names as on passport. Don't include titles (Mr, Mrs, Dr).

Full Name in Native Alphabet

If your name uses non-Latin characters (Chinese, Arabic, Hindi, etc.), enter it here. Otherwise check "Does Not Apply."

Have you ever used other names?

Include maiden names, previous married names, nicknames used on official documents, or aliases.

Sex / Date of Birth / City of Birth

Match your passport exactly. City of birth should match what's on your passport.

⚠️ Important: Every answer must match your passport exactly. Even small differences (like "Mohammad" vs "Mohammed") can cause problems.
3

Personal Information 2

Nationality

Your current nationality. If you hold multiple citizenships, list them all.

National Identification Number

Your national ID number (Aadhaar for India, NIE for Spain, BSN for Netherlands, etc.). If none, check "Does Not Apply."

US Social Security Number

Only if you have one from previous US employment. Otherwise "Does Not Apply."

US Taxpayer ID

Only if you have an ITIN. Otherwise "Does Not Apply."

4

Travel Information

Purpose of Trip

Select your visa category: B1 (business), B2 (tourism), F1 (student), H1B (work), etc. This determines which additional questions you'll see.

Specific Travel Plans

If "Yes": enter arrival date, departure date, arrival city, and addresses where you'll stay. If "No": enter approximate dates and intended length of stay.

US Address

Enter the hotel address or the address of the person you're visiting. Must be a real US address.

Who is Paying for Your Trip?

Options: Self, Other Person, Other Company/Organization. If someone else is paying, you'll need their details.

💡 Tip: If you haven't booked flights yet, use approximate dates. The dates don't need to match your actual booking exactly, but should be reasonable.
5

Travel Companions

Are you traveling with others?

If traveling with family/group, list their names. Each person needs their own DS-160.

Have you been to the US before?

If yes, provide dates, visa type, and length of stay. Be accurate — they can verify this.

Have you been refused a US visa?

Be honest. Lying is worse than a previous refusal. Explain briefly if asked at the interview.

6

US Contact & Address

Contact Person in the US

Name, organization, phone, email of someone in the US. Can be a hotel, friend, business contact, or university.

Relationship

Friend, Business Associate, School, Hotel, Relative, etc.

💡 If you don't know anyone in the US, use your hotel's name and address as the contact.
7

Passport Information

Passport Number

Enter exactly as printed. Include all letters and numbers. Double-check for O vs 0 and I vs 1.

Passport Book Number

Some passports have this on the data page. If yours doesn't, check "Does Not Apply." (Most Indian passports have this; most European passports don't.)

Issue Date / Expiry Date / Issuing Authority

Copy exactly from your passport.

⚠️ Your passport must be valid for at least 6 months beyond your intended stay in the US.
8

Employment & Education

Present Occupation

Select from the dropdown. If employed, provide employer name, address, phone, job title, start date, monthly salary.

Previous Employment

List your last employer if you changed jobs in the last 5 years.

Education

Highest level of education, institution name, address, course of study, dates attended.

9

Security Questions

These are yes/no questions about criminal history, immigration violations, terrorism, etc. For most applicants, all answers are "No."

⚠️ Answer honestly. Lying on the DS-160 is a federal crime and grounds for permanent visa ineligibility. If you have a complicated history, consult an immigration attorney before applying.
10
